Paradoxically, the onset of treatment to decrease serum uric acid levels is associated with acute gout flare. While the mechanism behind this phenomenon is unknown, it has been hypothesized that the initial lowering of serum urate may activate previously precipitated crystals.

However, in patients with severe impairment, the starting dose should be 0.3 mg/day and any increase in dose should be done with close monitoring. For control of acute attacks of joint pain, there are NSAIDs, colchicine and corticosteroids. After gout flares have resolved, there are medications that can lower the level of uric acid over time in order to prevent or lessen attacks. Treatment of the acute gout attack in patients with renal insufficiency has been recently reviewed.215 NSAIDs are frequently not an option in such cases.

Based on these findings, these investigators performed a pilot, open-labeled study in 10 patients with gout who could not tolerate or had failed standard anti-inflammatory therapies. All 10 patients with acute gout responded rapidly to anakinra. Blockade of IL-1 appears to be an effective therapy for acute gouty arthritis. The authors stated that these findings need to be confirmed in a controlled study. In this elderly lady with chronic tophaceous gout, significant renal impairment, and other comorbidities, we started urate lowering therapy with 80 mg of febuxostat q.d.

In patients not treated with urate-lowering drugs, 62% will experience a recurrence within 1 year of the initial attack, 78% by 2 years, and 89% by 5 years.200 For patient comfort, it is important to reduce or eliminate gouty flares. Patient and physician education that includes dietary trigger modifications is an important element in both gout flare prophylaxis and long-term gout management. Education must be manageable and aimed at behavioral changes in exercise, diet/alcohol intake, and treatment adherence, along with awareness of what the overall management plan is. Strategies are based on medications that control the rapid development of inflammation that occurs when microtophi or tophi are destabilized within the joint. Gout attack prophylaxis encompasses the time period from second attack through the start and escalation of ULT until at least 3 months after target SUA levels are reached. Previous corticosteroid injections may confuse the diagnosis because they appear as strongly positive birefringent crystals.

People taking alpha-blockers may also use less pain medication and may not need to visit the hospital. Alpha-blockers appear to be more effective for larger stones than smaller stones. However, use of alpha-blockers may be associated with a slight increase in serious, unwanted effects from this medication. A combination of tamsulosin and a corticosteroid may be better than tamsulosin alone.
