Administering XOIs or uricosuric agents during an acute gout flare may worsen symptoms by mobilizing urate crystals. Anti-inflammatory prophylaxis with colchicine, NSAIDs, or glucocorticoids must be administered before initiating ULT. For those with a higher level, for example, 10.0 mg/dL, diet alone will not usually prevent gout. For the latter, even a very strict diet only reduces the blood uric acid by about 1 mg/dL- not enough, in general, to keep uric acid from precipitating in the joints.
