Diagnosis And Management Of Gout
Content
There is a perception of gout as an acute disease requiring treatment only for acute flares. However, to combat the disease, chronic ULT, reducing SU levels to below the saturation threshold (6.8 mg/dL), and chronic anti-inflammatory prophylaxis, especially during ULT initiation, are needed. The awareness of the importance of patient perspectives on gout treatment may be growing.
Secondary outcomes included the improvement of primary joint pain , tenderness, swelling, treatment response and patient global assessment of wellbeing across days 1–5. The number of patients achieving ⩾ 50% decrease in NRS pain scores following baseline on days 2–5 were also compared between treatment arms. Other outcomes included the number and type of AE that occurred during the first 7 days of the study, as well as the decrease in CRP levels after 7 days. Finally, use of concomitant pain-relieving agents, both prescription-based and over the counter medication, during days 1–7 were determined.

Obesity, insulin resistance, high cholesterol, heart disease, hypothyroidism, and kidney disease can be associated with gout. Episodes of gout have been noted after injury or surgery, sometimes involving infection or the use of contrast for x-rays. This procedure involves removing the painful joint and replacing it with artificial parts. The goal of joint replacement is to provide pain relief, as well as to maintain joint movement.
He or she may have a personal or family track record of gout and acute attacks in the hospital. The medication history may signal red flags, as well, such as chronic gout therapy that was discontinued on admission. The disease is linked to obesity, metabolic syndrome, diabetes, heart failure, cardiovascular disease, hyperlipidemia, diuretic use, hypertension, and kidney disease, so any of those can be an indicator. These medications decrease the total amount of uric acid in the body and lower the serum uric acid level. For most patients, the goal of uric-acid-lowering medication is to achieve a serum uric acid level of less than 6 mg/dl. These medications also are effective treatments to decrease the size of tophi, with the ultimate goal of eradicating them.
Examples include 500 mg of naproxen twice a day or 50 mg of indomethacin three times per day, Dr. O’Dell said. Swift diagnosis with aspiration is essential for septic joint, so the patient can begin antibiotic treatment immediately. Staphylococcus aureus is one of the most common causes of septic joints in otherwise healthy patients and destroys joint cartilage very quickly if not treated right away, he added. While some people with gout benefit from cutting back on foods that are high in purine, a chemical that contributes to uric acid formation, it is not clear how much purine intake makes a difference. Moderate changes to your eating style may help you feel better and reduce gout risks, and research suggests that purine-rich vegetables don’t trigger gout. High-purine foods such as lentils and beans can be a smart source of lean protein.
Chronic Gouty Arthritis
In May 2020, the American College of Rheumatology updated its guidelines for gout treatment for the first time in eight years. There is more evidence in support of early diagnosis and treatment ;treat-to-target protocols; andallopurinol as a first-line agent. People with gout typically experience flare-ups, or attacks, of symptoms followed by periods with no symptoms.
What is the best thing to drink if you have gout?
Drink plenty of water, milk and tart cherry juice. Drinking coffee seems to help as well. Be sure to talk with your doctor before making any dietary changes.
Probenecid appears to be less effective than allopurinol and is a second line agent. Probenecid may be used if undersecretion of uric acid is present (24-hour urine uric acid less than 800 mg). It is, however, not recommended if a person has a history of kidney stones. Pegloticase is an option for the 3% of people who are intolerant to other medications. Pegloticase is given as an intravenous infusion every two weeks, and reduces uric acid levels.
Systemic corticosteroids have been used for the treatment of acute gout since 1952. As with other treatments for gout, systematic studies of the efficacy of corticosteroids – both systemic therapy and intra-articular therapy – are lacking. However, at least at relatively large doses, systemic corticosteroid therapy appears to be associated with good responses and few important side effects. Data suggest that generic NSAIDs can provide marked symptomatic relief within 24 hours of administration and may also provide substantial cost savings over other treatments for acute gout. NSAIDs have therefore become the drug of first choice in patients with acute attacks of gout who do not have underlying contraindications against their use.
Network meta-analysis including both direct and indirect evidence was performed by using a Bayes method. SMDs and RRs of network meta-analysis were also computed along with their 95% CIs. The authors of included studies with missing data will be contacted to get data. If the missing data is not accessible, we will exclude these articles and synthesis the rest of the included studies. Efficacy data will be synthesized and statistically analyzed in R3.5.1.

The current evidence confirms the effectiveness and safety of several drug interventions in the treatment of acute flares of gout, however, the most preferred drugs are still unclear. We, therefore, seek to conduct a network meta-analysis that can systematically compare non-steroidal anti-inflammatory drugs , COXIBs, colchicine, hormones, or IL-1 receptor antagonists, etc. for acute gout based on the latest evidence. Acute gout is traditionally treated with NSAIDs, corticosteroids, and colchicine; however, subjects have multiple comorbidities that limit the use of some conventional therapies.
Without treatment, an acute gout attack will be at its worst between 12 and 24 hours after it began. A person can expect to recover within 1 to 2 weeks without treatment, but there may be significant pain during this period. Medicine is considered the most effective treatment method for gout, but there are lifestyle changes you can make to manage your symptoms during a gout attack, or help prevent gout attacks from happening. Learn more about other ways to manage symptoms of gout during an attack here. If a patient can’t tolerate allopurinol, and meets the criteria for probenecid, that can be tried. In a clear-cut case, a primary care physician can make the diagnosis of gout with a high level of confidence.
Although essential for a normal metabolism, excess uric acid is excreted from the body mainly via the kidneys into urine. All published material, which is covered by copyright, represents the views of the contributor and does not reflect the opinion of the American College of Physicians or any other institution unless clearly stated. Dr. Fields agreed and noted that the decisions that physicians must make in their day-to-day practice cannot rely solely on the highest level of evidence from randomized controlled trials, particularly when those trials have not yet been conducted. He added that it's critical for physicians to know the quality of the evidence they are using to make clinical decisions. "However, we need to use all available evidence, along with expert opinion, to achieve optimal patient outcomes," he said.
Urate-lowering medications are the primary pharmacological intervention for managing hyperuricemia. In addition to following your doctor's treatment plan, there are choices you can make to prevent gout attacks and future joint damage. Although more expensive than allopurinol, febuxostat is an option for patients who cannot take probenecid or allopurinol.
Prophylactic medications are used during approximately the first six months of therapy with a medication to lower high levels of uric acid to either prevent gout flares or decrease the number and severity of flares. This is because any medication or intervention that either increases or decreases the uric acid level in the bloodstream can trigger a gout attack. By taking one of these prophylactic or preventative medications during the first six months of treatment with allopurinol, febuxostat, or probenecid, the risk of having a gout attack during this time is decreased.
Acute Attack Pain Management
Doctors who specialize in gout and other forms of arthritis are called rheumatologists. To find a provider near you, visit the database of rheumatologistsexternal icon on the American College of Rheumatology website. Once a rheumatologist has diagnosed and effectively treated your gout, a primary care provider can usually track your condition and help you manage your gout. Unlike allopurinol, which interacts with warfarin (Coumadin®), febuxostat did not have this interaction when studied. Febuxostat is approved by the FDA to start at 40mg daily, and if the uric acid has not reached goal (less than 6.0mg/dL) after two weeks of treatment the dose can be increased to 80mg daily.
