Pegloticase is given as an intravenous infusion every two weeks, and reduces uric acid levels. Pegloticase is useful decreasing tophi but has a high rate of side effects and many people develop resistance to it. Potential side effects include kidney stones, anemia and joint pain.

Participants treated with lesinurad plus allopurinol or febuxostat experienced reduced serum uric acid levels compared with placebo. The CONFIRMS trial demonstrated the efficacy and safety of febuxostat in lowering hyperuricemia. Treatment For Acute Flares Of Gout

Other conditions that can look similar include CPPD , rheumatoid arthritis, psoriatic arthritis, palindromic rheumatism, and reactive arthritis. Gouty tophi, in particular when not located in a joint, can be mistaken for basal cell carcinoma or other neoplasms. Data suggest that generic NSAIDs can provide marked symptomatic relief within 24 hours of administration and may also provide substantial cost savings over other treatments for acute gout. NSAIDs have therefore become the drug of first choice in patients with acute attacks of gout who do not have underlying contraindications against their use.

Another possible first-line therapy for an acute gout attack is adrenocorticotropic hormone, or corticotropin or ACTH (Acthar®, H.P. Acthar Gel®). It’s not a common first-line treatment for a gout attack, but it’s a possible option for some people, particularly those who cannot take oral NSAIDs, colchicine or corticosteroids for some reason, or who are hospitalized. Colchicine (Colcrys® or Mitigare®) is a prescription, oral anti-inflammatory medication that’s prescribed to treat gout and other types of crystal-related arthritis. There are some unusual conditions that colchicine also treats, but it’s not an anti-inflammatory medication that works for things like headaches or joint injury. Colchicine may be the first choice to treat your acute gout flare and ease your symptoms if it’s within 36 hours of the onset of your symptoms. Lowering the amount of uric acid in the blood is key to reducing the risk of a flare.

This occurs from a combination of diet, other health problems, and genetic factors. At high levels, uric acid crystallizes and the crystals deposit in joints, tendons, and surrounding tissues, resulting in an attack of gout. Gout occurs more commonly in those who regularly drink beer or sugar-sweetened beverages or who eat foods that are high in purines such as liver, shellfish, or anchovies, or are overweight. Diagnosis of gout may be confirmed by the presence of crystals in the joint fluid or in a deposit outside the joint.

Because of this, the patient’s workup was complex, reflecting a broad differential diagnosis. He eventually provided sufficient descriptions of prior episodes of bilateral upper extremity swelling and pain that allowed them to be clearly differentiated from his gout attacks. These episodes were more consistent with CECS, with the worst case being the episode of ACS that we report here.

Epidemiologic studies, however, have not been consistent in using universal investigative methods.
