Effective treatments for gout attacks include NSAIDs, colchicine, and corticosteroids (high SOE; see Table 2). The two systematic reviews summarized here assessed the accuracy of tests to diagnose gout and also examined the evidence regarding the treatment of gout in the primary care setting. The diagnosis for gout is made based on a physical examination by your doctor, x-rays, and lab tests. You will be asked about your symptoms and how gout has changed your activities. Because medications and other diseases can cause gout, you will be asked to provide a medical history and a medication list.

Other Treatments
Corticosteroids are typically reserved for patients who cannot tolerate NSAIDs or colchicine, or those who cannot use such medications because they have a medical condition which prohibits their use. In addition to medications for an acute attack, application of ice to the affected joint may be helpful to reduce pain. Currently there is no evidence regarding whether NSAIDs or low-dose colchicine is the best treatment for acute gout. This trial will be the first direct comparison of the effectiveness and side effects of a NSAID and low-dose colchicine to treat acute gout in primary care. Naproxen will be used in this trial because it has been shown to be as effective as oral prednisolone for the treatment of acute gout, is safer than other commonly used NSAIDs such as diclofenac and indomethacin, and is inexpensive.
While medications like naproxen or colchicine can reduce pain and inflammation during an acute gout flare-up, they don’t do anything to address the underlying problem. That’s where urate-lowering medication like allupurinol comes in. Overall, purine restriction generally reduces serum uric acid levels by no more than 1 mg/mL, with modest impact, and diets with very low purine content are not palatable.
Chronic Gout
Finally, treatment recommendations for acute and chronic gout are suggested. Without treatment, an acute attack of gout usually resolves in five to seven days; however, 60% of people have a second attack within one year. Those with gout are at increased risk of hypertension, diabetes mellitus, metabolic syndrome, and kidney and cardiovascular disease and thus are at increased risk of death. It is unclear whether medications that lower urate affect cardiovascular disease risks.
Does soaking in hot water help gout?
Soaking in cold water is most often recommended and considered most effective. Ice packs may also work. Soaking in hot water is typically only recommended when inflammation isn't as intense. Alternating hot and cold applications may also be helpful.

Corticosteroids such as prednisone, methylprednisolone , and prednisolone , also can be used during an acute gouty flare. However, the total dose of steroids is generally limited due to potential side effects such as cataract formation and bone loss. Steroid medications are extremely helpful in treating gout flares in patients who are unable to take colchicine or NSAIDs. Hypertension reduces renal excretion of urate4,30 leading to hyperuricemia.
Urgent Ortho Care
There are a number of possible consequences of this buildup of uric acid in the body, including acute and chronic gouty arthritis, kidney stones, and local deposits of uric acid in the skin and other tissues. Symptoms
Although exercise is great for helping lose weight and is generally fine for gout patients, when you have a gout flare in your toe, foot, ankle or knee it is advised to stay off the foot as much as possible until the flare resolves. It’s fine to do other kinds of exercise, for example any exercise involving the upper body, but give the gouty joint a rest. This is another reason to treat gout flares quickly, since starting early often means the flare will be short – and you can limit your time off your feet.
