Is a heating pad good for gout?
Research shows that ice is particularly soothing for gout; if you're dealing with a different type of arthritis, a heating pad may be a better choice. Some people also say that eating cherries or drinking tart cherry juice helps during a gout flare.
Medications available to control pain and inflammation include nonsteroidal anti-inflammatory drugs , colchicine, and corticosteroids. The customary treatment of choice for an acute gout attack is an NSAID. Although indomethacin is the NSAID generally used, all NSAIDs can be effective in treating a gout flare-up. Some patients, including those with heart failure, liver disease, ulcers, or improperly working kidneys, those taking medications called anticoagulants , and the elderly, cannot take NSAIDs. EULAR guidelines suggest that allopurinol be used to lower uric acid levels for the long term.
About Gout
The pain from a gout attack usually gets better in 3 to 10 days. An exam and tests will show if it’s gout or something else, like an infection. It usually affects one joint at a time, but gout can spread to other joints and leave them looking red and swollen. When the flare starts, most people have redness, swelling, and severe pain, usually in one joint. The most common place for gout is the base of the big toe, but it can happen in other joints such as the elbow, knee, wrist, ankle, and instep.
The crystallization of uric acid, often related to relatively high levels in the blood, is the underlying cause of gout. This can occur because of diet, genetic predisposition, or underexcretion of urate, the salts of uric acid. Underexcretion of uric acid by the kidney is the primary cause of hyperuricemia in about 90% of cases, while overproduction is the cause in less than 10%.
Acute Attack Pain Management
Gout is due to persistently elevated levels of uric acid in the blood. This occurs from a combination of diet, other health problems, and genetic factors. At high levels, uric acid crystallizes and the crystals deposit in joints, tendons, and surrounding tissues, resulting in an attack of gout. Gout occurs more commonly in those who regularly drink beer or sugar-sweetened beverages or who eat foods that are high in purines such as liver, shellfish, or anchovies, or are overweight.
Current data supports the administration of colchicine after achieving a serum uric acid level less than 6 mg/dl after 3 months in patients without tophi and 6 months in patients with tophi. The recommended dose in patients with ESRD is 0.3 mg twice a week. Sometimes during an acute gout attack, uric acid levels may test normal because the uric acid has left your bloodstream and entered the inflamed tissue. If you do have a high uric acid level during a gout attack, it is likely that the level was even higher before the attack. To help diagnose gout, your doctor may check your blood uric acid levels in between attacks to see if they run high.
However, patients also may arrive in the emergency department with an acute attack. In patients with gout who have moderate to severe kidney disease, ACR guidelines recommend xanthine oxidase inhibitor therapy with either allopurinol or febuxostat as the first-line pharmacologic approach. Probenecid can be used in patients who have contraindications to or are intolerant of at least 1 of those first-line agents, or it may be combined with a xanthine oxidase inhibitor if the inhibitor does not lower uric acid sufficiently. Probenecid could also be used for those patients who consider the risks of xanthine oxidase inhibitors to be too high.
Transaminase levels can become elevated and should be measured periodically. Gouty arthritis can cause pain, deformity, and limited joint motion. Inflammation can be flaring in some joints while subsiding in others.
Lesinurad is taken once a day, so more convenient than probenecid. Lesinurad is now available in combination with allopurinol, allowing a person taking both medications to take a single pill a day. The combination pill is marketed as Duzallo®, which comes as either a combination of allopurinol 300mg with 200mg of lesinurad or a combination of 200mg allopurinol and 200mg lesinurad. Although exercise is great for helping lose weight and is generally fine for gout patients, when you have a gout flare in your toe, foot, ankle or knee it is advised to stay off the foot as much as possible until the flare resolves. It’s fine to do other kinds of exercise, for example any exercise involving the upper body, but give the gouty joint a rest. This is another reason to treat gout flares quickly, since starting early often means the flare will be short – and you can limit your time off your feet.
Signs And Symptoms Of Gout
Acute attacks of gout are excruciatingly painful and require urgent drug treatment to reduce inflammation, most commonly with antiinflammatory drugs or colchicine. In primary care, NSAIDs are most commonly used but can cause serious side effects such as stomach ulcers and heart disease, particularly in the elderly. Patients frequently require repeat prescriptions for recurrent attacks of acute gout increasing the risk of drug-related side-effects.
Can Dietary Changes Prevent Gout?
Fortunately, there are medications to help prevent gout attacks when you have chronic gout. While historically it is not recommended to start allopurinol during an acute attack of gout, this practice appears acceptable. Allopurinol blocks uric acid production, and is the most commonly used agent. Long term therapy is safe and well-tolerated and can be used in people with renal impairment or urate stones, although hypersensitivity occurs in a small number of individuals.

A gout-friendly diet should include two tablespoons of nuts and seeds every day. Good sources of low-purine nuts and seeds include walnuts, almonds, flaxseeds and cashew nuts.
The IL-1 receptor antagonist anakinra, approved for rheumatoid arthritis, was studied in 10 patients with acute and polyarticular gout, resolving the attack in 9 patients without adverse effects . Rilonacept, a soluble IL-1 receptor-Fc fusion protein, was evaluated in 10 refractory gout patients, resulting in decrease of pain and C-reactive protein. A good safety profile was shown; however, no improvement in the number of affected joints was demonstrated . Canakinumab is an IL-1 fully human monoclonal antibody investigated in a phase 2 study that involved 200 patients with acute gout. An intramuscular injection of 150 mg canakinumab was superior to triamcinolone acetonide, and no toxicity or infections were observed .

Febuxostat side effects include rash, nausea and reduced liver function. Febuxostat also may increase the risk of heart-related death. Patients should be treated with NSAIDs, systemic corticosteroids, or low-dose oral colchicine during an acute attack, which is another point of agreement between the ACP and the ACR 2012 and EULAR 2016 guidelines. Dr. McLean agreed that the ACR-EULAR gout classification criteria, as well as some clinical algorithms, can be used to help guide clinical judgment. However, he said, "there is insufficient evidence for us to clearly endorse a single one of those tools at this time." Infusion therapy is an ideal alternative for patients for whom swallowing medication is difficult, uncomfortable, or impossible — often due to their chronic health conditions.

The question of surgery for gout most commonly comes up when a patient has a large clump of urate crystals , which is causing problems. This may be if the tophus is on the bottom of the foot, and the person has difficulty walking on it, or on the side of the foot making it hard to wear shoes. An especially difficult problem is when the urate crystals inside the tophus break out to the skin surface. This then can allow bacteria a point of entry, which can lead to infection, which could even track back to the bone.
