In the case of glucocorticoid use, all studies found described either refractory or very severe gout flare cases, which are not necessarily reflecting the common clinical practice of gout flare management. We did not find studies exploring the question of whether low doses of glucocorticoids could be part of the prophylaxis of gout flares. Another question that remains is whether glucocorticoid use is equally effective and safe or if there is a potential risk of exacerbating tophaceous gout disease. Gout is an inflammatory crystal arthropathy caused by the precipitation and deposition of uric acid crystals in synovial fluid and tissues. Decreased renal excretion and/or increased production of uric acid leads to hyperuricemia, which is commonly asymptomatic but also predisposes to gout.
How Uric Acid Crystals Form
Factors that influence rates of gout include age, race, and the season of the year. Studies in the early 2000s found that other dietary factors are not relevant. Specifically, a diet with moderate purine-rich vegetables (e.g., beans, peas, lentils, and spinach) is not associated with gout. Alcohol consumption is strongly associated with increased risk, with wine presenting somewhat less of a risk than beer or spirits.

Mean peak plasma levels and AUC of colchicine were two times higher in elderly subjects compared to young healthy males.
Other complementary drugs may be used in gout treatment, including Cozaar , an antihypertensive drug, and Tricor , a lipid-lowering drug. Ultrasonography is more sensitive (although operator-dependent) and specific than plain x-rays for the diagnosis of gout. Urate deposition over the articular cartilage (double-contour sign) and clinically inapparent tophi are characteristic changes.

In the case of our patient, urate lowering therapy was initiated early, in a proper dose and with good compliance. Therefore, we believe that particular factors related to gout pathophysiology and/or particular metabolic mechanisms may have played an essential role in this patient, resulting in a difficult to treat attack, despite adequate therapy.
