Gout Treatment Is Questioned In New Guidelines

While the chronic use of colchicine can reduce the attacks of gout, it does not prevent the accumulation of uric acid that can lead to joint damage even without attacks of hot, swollen joints. Like allopurinol, the most common side-effect of febuxostat is causing gout to flare after this drug is started. As with allopurinol, it is reasonable whenever possible to add a preventative medication, such as colchicine, for at least the first six months after starting febuxostat to help avoid gout flares.

When the two agents are used together, the increase in uric acid levels has been determined to be insignificant. No dose adjustment is necessary when febuxostat is used with HCT, warfarin (Coumadin, Bristol-Myers Squibb), or desipramine (Norpramin, Sanofi-Aventis). The primary endpoint was the proportion of patients with the last three monthly serum urate levels of below 6 mg/dL.
