#112 Gout Flares

Febuxostat and pegloticase are now available and other new therapeutics are in the pipeline. Initiation of the inflammatory response to monosodium urate crystals by the innate immune system. MSU are recognized as danger-associated molecular patterns by innate immune receptors, including Toll-like receptors , on monocytes. One pathway, involving the adapter protein used by TLRs, MyD88 , leads to translocation of activated NFκ-β (nuclear factor κ-β) to the nucleus where it activates transcription of pro-IL-1β.

Febuxostat and allopurinol are equally effective in preventing recurrent gout. Causes Of Gout Flares

Some drugs such as HMG-CoA reductase inhibitors and fibrates may increase the risk of myopathy when combined with colchicine capsules. Complaints of muscle pain or weakness could be an indication to check serum creatinine kinase levels for signs of myopathy. Physicians should ensure that patients are suitable candidates for treatment with colchicine capsules and remain alert for signs and symptoms of toxic reactions associated with increased colchicine exposure due to drug interactions.

These 8 studies of IL-1 inhibitor use as gout flare therapy included 4 case series and 3 case reports of anakinra use, and 1 case report of canakinumab use. A standard 100-mg dosing was routinely observed in studies of anakinra use, albeit varying dose frequency and duration in the context of varying degree of renal impairment and the duration of gout flare. Six out of 7 studies of anakinra use demonstrated stable renal function during treatment irrespective of pre-existing CKD .

What Does It Mean To Have Gout?

Triggers for acute attacks (flare-ups) of gout include surgery, dehydration, beverages sweetened with sugar or high fructose corn syrup, beer, liquor, red meat, and seafood. Gouty arthritis is caused by the deposition of needle-like crystals of uric acid in a joint. Talk to your doctor about ways to prevent gout attacks during your sleep. Gout attacks happen more at night and in the early morning rather than during the day. The reasons this happens are not entirely known, but some of the leading ideas are dehydration, lower body temperature, and changes in hormone levels during sleep.

Colchicine dose needs to be adjusted in patients with significantly decreased kidney function. Colchicine has interactions with certain other medications, most notably clarithromycin (Biaxin®). Avoidance of purine rich foods and alcohol may help lower uric acid levels and prevent significant fluctuations in serum uric acid that may precipitate acute attacks. Obesity and increased fat distribution are risk factors for gout.

Other Treatments

Several lifestyle-dependent factors such as alcohol consumption, meat consumption and a high BMI increase the risk of gout. Interestingly, three injection site reactions were reported in the treatment as usual group with the placebo injections, compared with none in the anakinra treatment group. One patient in the treatment as usual group reported to have viral gastroenteritis , and one patient in the anakinra group had a respiratory tract infection . This randomized, double-blind, double-dummy, active comparator placebo-controlled trial, was conducted at the rheumatology departments of seven hospitals in the Netherlands, between 2016 and 2018.

The doctor can help the person determine the best course of treatment, which is likely to include dietary changes and home remedies. Treatments focus on reducing symptom severity during a flare-up and preventing future flare-ups. Doctors typically aim to reduce the amount of uric acid in the blood, which contributes to gout symptoms.

Anti-inflammatory steroids have long-term risks, such as bone thinning and infection, but their risk for short-term (for example, 3-7 days) therapy is relatively low. These agents can raise blood pressure and blood sugar, so can be a problem for those with uncontrolled hypertension or uncontrolled diabetes mellitus. A third type of crystal-induced arthritis, hydroxyapatite deposition disease, has a type of crystal that needs special studies for identification.

What can you not eat with gout?

Bananas are low in purines and high in vitamin C, which make them a good food to eat if you have gout. Changing your diet to include more low-purine foods, like bananas, can lower the amount of uric acid in your blood and reduce your risk of recurrent gout attacks.

THZ, XGX, and LFR contributed to the conception and design of the study protocol. The search strategy was developed and run by YZH and THZ, who will also screen the title and abstract of the studies after running the search strategy. XGX and ZQH will screen full copies of the remaining studies after the title and abstract selection. YZH and XQW will extract information from the included studies and enter into the electronic database. All the authors drafted and revised this study protocol and approved it for publication.
