Combining these dual inhibitors with colchicine in patients with renal or hepatic impairment has resulted in life-threatening or fatal colchicine toxicity. Four pharmacokinetic studies evaluated the effects of co-administration of voriconazole , fluconazole , cimetidine , and propafenone on systemic levels of colchicine. Colchicine can be administered with these drugs at the tested doses without a need for dose adjustment. Gastrointestinal disorders are the most common adverse reactions with colchicine.
Gout Attack Vs Chronic Gout
In the context of minimal or absence of residual renal function, treatment options for gout flare are limited, with potential risks of further renal impairment. Renally adjusted dosing is often required in people with CKD, although there is no specific evidence-based guidance in monitoring the efficacy and safety of the treatment used. Therefore, clinicians often remain judicious when facing this common clinical conundrum in the management of gout flares. It is reassuring that, based on a recent systematic review, colchicine use is relatively safe in all possible clinical indications, with diarrhoea and gastrointestinal symptoms being the most commonly reported adverse events . However, no conclusion could be precisely drawn from this review on the safety profile of colchicine use in people with CKD . Overall, for all anti-inflammatory drugs used for gout flare, lack of consensus on the appropriate dosing and treatment monitoring for this high-risk comorbid population remains.
Some of the underlying cardiometabolic comorbid conditions are themselves independent risk factors for flares, which further contribute to the complexity of treatment of gout flares. In conclusion, in our study anakinra was shown to be non-inferior to registered treatment as usual for the treatment of acute gout flares. Our results suggest that anakinra is an effective treatment alternative for the treatment of acute gout flares. By incorporating standardised gout flare definition in future gout flare studies, comparisons in treatment outcomes across studies of different treatments used as gout flare prophylaxis and therapy can be performed fairly and efficiently. Ideally, the efficacy and safety of gout flare and urate-lowering treatments based on stratified renal function should be emphasised in all gout studies, as gout and CKD often co-exist.
Losartan is an alternate antihypertensive medication and fenofibrate is a lipid-lowering agent that have mild uricosuric activity, and may be useful adjuncts in this population [Wurzner et al. 2001; Yamamoto et al. 2001]. Unfortunately, even in typical patients accurately diagnosed, good outcomes may be elusive. Improper prescribing or poor compliance is the usual cause of urate-lowering therapy failure.

ABCG2 is a urate-transporter gene involved mainly in urate excretion. However, there is currently not enough data to support the clinical use of the ABCG2 gene variant. Although the manufacturer of anakinra has not sought FDA approval for a gout indication, the drug is utilized by rheumatologists, particularly in hospitalized patients with co-morbidities.
