Gout may later affect several joints, including those that may have been free of symptoms at the start of the disorder. In rare cases, the shoulders, hips, or spine are affected. Gouty arthritis was among the earliest diseases to be recognized as a clinical entity.
Is gout a permanent condition?
The pain and inflammation associated with gout attacks typically disappear completely between attacks. But repeated attacks of acute gout can cause more permanent damage.
Uric acid also can deposit in the urinary tract, causing kidney stones. Some patients only experience acute gout attacks which may be limited to 1-2 times per year (or even 1-2 times in lifetime). However, for some patients, gout can be a chronic, relapsing problem with multiple severe attacks that occur at short intervals and without complete resolution of inflammation between attacks.

This pain is like that of a dislocation, and yet the parts feel as if cold water were poured over them. After a time this comes to a full height, accommodating itself to the bones and ligaments of the tarsus and metatarsus. Now it is a violent stretching and tearing of the ligaments– now it is a gnawing pain and now a pressure and tightening.
Hyperuricemic therapy should be initiated in patients with frequent gout attacks, tophi or urate nephropathy. A low dosage of an NSAID or colchicine is effective in preventing acute gouty attacks. Hyperuricemic drug therapy should not be started until an acute attack of gouty arthritis has ended, because of the risk of increased mobilization of uric acid stores. A reasonable goal is to reduce the serum uric acid concentration to less than 6 mg per dL (360 μmol per L). The pain and swelling of gout are caused by uric acid crystals that deposit in the joint. Uric acid is a substance that normally forms when the body breaks down waste products called purines.

This purine analogue of hypoxanthine, along with its active metabolite oxypurinol, competitively inhibits xanthine oxidase, causing a decrease in production of urate. The usual dosage is 300 mg/d, typically starting at 100 mg/d and slowly increasing over a period of weeks. Colchicine traditionally was given up to 6 times per day, but this dosing regimen has fallen out of favor because of the difficulty patients have in tolerating the diarrhea that may occur.

After the most severe pain subsides, some joint discomfort may last from a few days to a few weeks. Later attacks are likely to last longer and affect more joints. Developed countries have a higher burden of gout than developing countries.

Lowering uric acid levels with medication can prevent future gout flares and long-term complications that go with them. Gout Symptoms
Formulations injected include methylprednisolone acetate (Depo-Medrol®), triamcinolone (Aristospan®), and betamethasone (Celestone®). Of these preparations, betamethasone lasts the shortest time in the joint of these preparations, but gout tends to be self-limited within a few weeks, in any case, so this option can be quite successful. The advantage of betamethasone is a decreased likelihood of temporarily worsened flares the day after the injection, which is the most common adverse reaction to local steroid injections. In the past, high doses of colchicine were used for gout attacks, but this tended to cause diarrhea in a large number of patients. It has been shown that lower doses of colchicine are as effective as high doses for an attack of gout, and much better tolerated.
The symptoms of gout and the inflammatory process usually resolve in three to ten days with treatment. If gout symptoms continue despite the initial treatment, or if repeated attacks occur, see your primary care physician for maintenance treatment that may involve daily medication. In cases of repeated episodes, the underlying problem must be addressed, as the buildup of uric acid over time can cause arthritic damage to the joint.

Taking folic acid and vitamin C may reduce uric acid level. Rilonacept (IL-1Trap) injection is being studied to see if it can help reduce gout flares during initiation of uric acid-lowering therapy with allopurinol. More research is necessary to confirm the early positive results, its long-term safety, and the patients most likely to benefit.
Chronic Gout
Assuming no other medical problems that require an adjusted dose, for an attack of gout a patient would receive two tablets of colchicine, 0.6mg each, as soon as possible after a gout attack starts. They would then receive one additional tablet an hour later. Colchicine dose needs to be adjusted in patients with significantly decreased kidney function. Colchicine has interactions with certain other medications, most notably clarithromycin (Biaxin®).
