Intravenous use can be very effective, and doesn't cause diarrhea by this route, but this agent must be given extremely carefully, since an error in dosing can shut down the bone marrow’s production of blood cells, and potentially be fatal. For this reason, intravenous colchicine is very rarely used today. Patients often ask about why colchicine, which has been available in unbranded form for many years, is now a branded drug (Colcrys®, Mitigare®). Pathophysiology Of Gout
Colchicine use is associated with decreased prevalence of myocardial infarction in patients with gout. Patients with GI bleeding or a history of peptic ulcer disease should avoid NSAID use because of increased bleeding risk. If an NSAID is used, proton pump inhibitors decrease the risk of NSAID-associated mucosal damage.

Dosing every 2–6 hours has long been known to reduce side effects [Kim et al. 2003]. This approach was validated in a randomized controlled trial of 184 patients with acute gout flares of less than 12 hours duration comparing low-dose to high-dose colchicine (4.8 mg total over 6 hours) and placebo [Terkeltaub et al. 2010]. The low-dose approach resulted in comparable peak plasma concentrations and efficacy (50% reduction in pain within 24 hours) with a side-effect profile similar to placebo. However, over 30% of patients failed to achieve the primary endpoint in both treatment arms, and colchicine dosing for attacks of greater duration remains unclear. Long-term prophylaxis with urate lowering therapy is used to maintain serum urate levels below 6 mg/dL, while considering the individual needs of the patient. Xanthine oxidase inhibitors , uricosuric, and uricase agents are three classes of drugs approved for lowering urate levels to help prevent acute flares and development of tophi in patients with gout.
Complications Of Gout

Sometimes other tests, such as an RF or an ANA (anti-nuclear antibody), may be ordered to rule out other causes of arthritis symptoms.
A minimum of 32 patients completing the study are needed for a meaningful conclusion. Physiologically, during day time, the synovial fluid is able to maintain a constant concentration of intra-articular monosodium urate crystals. At night, there is an articular dehydration attributed to joint rest, causing the passage of interstitial fluid to the circulatory system. In this process, oversaturation of synovial fluid with MSU crystals occurs, leading to nocturnal onset of attacks.
What Is The Difference Between Acute And Chronic Gout?
The ACR guidelines do not recommend pegloticase as a first-line approach. Nonrecombinant urate-oxidase is used in Europe to prevent severe hyperuricemia induced by chemotherapy in patients with malignancies, as well as in selected patients with treatment-refractory gout. Short-term use of such agents in patients with severe tophaceous gout could debulk the total-body urate load, allowing maintenance with probenecid or allopurinol. Given the elevated risk of severe cutaneous reactions to allopurinol in patients with heart disease, it is important to ensure that hyperuricemia is not related to heart disease medication before starting allopurinol. Rather than allopurinol, the patient may simply need modification of the cardiac regimen .
