They commonly develop in the fingers, hands, feet, the tough band extending from the calf muscles to the heel , or around the elbows. Typically during a flare, severe pain occurs suddenly in one or more joints, often at night. The pain becomes progressively worse and is often excruciating, particularly when the joint is moved or touched.
It is very unlikely that a patient receiving proper treatment would progress to this stage. Finally, doctors can search for urate crystals around joints or within a tophus using ultrasound scan. X-rays cannot detect gout, but may be used to rule out other causes.
The treatment of certain types of cancer can cause gout because of high levels of uric acid released when the cancer cells are destroyed. Degenerative arthritis also makes affected joints more likely to be the site of a gouty attack. Gout is a disease resulting from the deposition of urate crystals caused by the overproduction or underexcretion of uric acid. The disease is often, but not always, associated with elevated serum uric acid levels. Clinical manifestations include acute and chronic arthritis, tophi, interstitial renal disease and uric acid nephrolithiasis. The diagnosis is based on the identification of uric acid crystals in joints, tissues or body fluids.

A definitive diagnosis of gout is based upon the identification of monosodium urate crystals in synovial fluid or a tophus. All synovial fluid samples obtained from undiagnosed inflamed joints by arthrocentesis should be examined for these crystals. Under polarized light microscopy, they have a needle-like morphology and strong negative birefringence.
Most mammals, except humans, have an enzyme called uricase. Uricase breaks down uric acid so it can be easily removed from the body. Because humans lack uricase, uric acid is not easily removed and can build up in body tissues. Often, doctors advise patients to keep taking colchicine in a low, preventive dose together with the uric acid-lowering medicine for at least six months. Gout is often diagnosed upon the finding of uric acid based crystals. The physician may use a needle to extract fluid from an affected joint and will study that fluid under a microscope to find whether urate crystals are present.

Other blood tests commonly performed are white blood cell count, electrolytes, kidney function and erythrocyte sedimentation rate . However, both the white blood cells and ESR may be elevated due to gout in the absence of infection. A white blood cell count as high as 40.0×109/l (40,000/mm3) has been documented. Long-standing elevated uric acid levels may result in other symptoms, including hard, painless deposits of uric acid crystals known as tophi.

Other organ transplants, such as heart and liver, also increase the risk of developing gout. This is because the surgery itself raises the risk of developing gout, as does the medicine cyclosporine used to prevent rejection of the transplanted organ. Cyclosporine also interacts with indomethacin, a common gout medicine. Acute gouty arthritis occurs when the first symptoms of gout appear. For others, these agents are not sufficiently effective. Some of the more promising include anakinra, rilonacept, canakinumab, BCX4208 and arhalofenate. Diet has been discussed in more detail above, and gout is clearly one of the rheumatic diseases where diet is unequivocally important.
Is coffee bad for gout?
There's very little evidence that suggests coffee intake causes gout or increases the risk of a gout flare-up. Although the majority of evidence is in favor of drinking coffee to reduce gout risk, there's still room to continue to expand the research.
Acute gout attacks are characterized by a rapid onset of pain in the affected joint followed by warmth, swelling, reddish discoloration, and marked tenderness. The small joint at the base of the big toe is the most common site for an attack. Other joints that can be affected include the ankles, knees, wrists, fingers, and elbows. In some people, the acute pain is so intense that even a bed sheet touching the toe causes severe pain. These painful attacks usually subside in hours to days, with or without medication. Most people with gout will experience repeated bouts over the years.
