Hyperuricemia is defined as a plasma urate level greater than 420 μmol/l (7.0 mg/dl) in males and 360 μmol/l (6.0 mg/dl) in females. Other blood tests commonly performed are white blood cell count, electrolytes, kidney function and erythrocyte sedimentation rate . However, both the white blood cells and ESR may be elevated due to gout in the absence of infection. A white blood cell count as high as 40.0×109/l (40,000/mm3) has been documented.
Gout is rare in most other animals due to their ability to produce uricase, which breaks down uric acid. Humans and other great apes do not have this ability; thus, gout is common. Other animals with uricase include fish, amphibians and most non-primate mammals.
How Are Gout Attacks Prevented?
The diagnosis is based on the identification of uric acid crystals in joints, tissues or body fluids. Treatment goals include termination of the acute attack, prevention of recurrent attacks and prevention of complications associated with the deposition of urate crystals in tissues. Pharmacologic management remains the mainstay of treatment.
How painful is gout on a scale?
The pain during a gout flare is so excruciating that many visit the emergency room for care. On a typical pain scale, most people with gout will rank their pain as a nine or a 10 – with even the slightest touch causing agony.
It may get worse quickly, last for hours, and be so intense that even light pressure from a sheet is intolerable. The exact cause of hyperuricemia sometimes isn't known, although inherited factors seem to play a role. For some folks, a sharp pain in the night can be a troubling experience. Gout can present in several ways, although the most common is a recurrent attack of acute inflammatory arthritis . The metatarsal-phalangeal joint at the base of the big toe is affected most often, accounting for half of cases. Other joints, such as the heels, knees, wrists, and fingers, may also be affected. Joint pain usually begins during the night and peaks within 24 hours of onset. Other symptoms may rarely occur along with the joint pain, including fatigue and a high fever.

What causes gout in toes?
Gout occurs when urate crystals accumulate in your joint, causing the inflammation and intense pain of a gout attack. Urate crystals can form when you have high levels of uric acid in your blood. Your body produces uric acid when it breaks down purines — substances that are found naturally in your body.
Many gout-associated genes play a role in releasing urate into the urine if levels are too high or reabsorbing it back into the bloodstream if more is needed in the body. Other associated genes are involved in transporting or breaking down sugars or transporting other small molecules. Of all the genes that have been studied, two genes, SLC2A9 and ABCG2, seem to have the greatest influence on urate levels. Gout is caused by a combination of genetic and environmental factors. Some of the factors that contribute to this condition have been confirmed by research, while others are unknown. The main risk factor for developing gout is hyperuricemia.
Anakinra (brand name Kineret®) is a biologic medication that blocks the inflammatory protein IL1. This medication is injected subcutaneously by the patient once a day, usually for 3 days, but can be used longer if needed to resolve a flare. Although much data supports the effectiveness and safety of this medication for gout, it is expensive and not as yet FDA approved for gout flares.
