Who Should Diagnose And Treat Gout?

People who develop gout likely have some genetic predisposition combined with other risk factors or conditions — such as kidney disease or diabetes — that cause uric acid to build up in the body. In addition to treating an acute gout attack, preventively lowering uric acid levels with medication can prevent gout flares and the long-term complications that go with them. Medications called xanthine oxidase inhibitors limit the amount of uric acid your body produces.

What Does Gout Pain Feel Like?

When the body has high levels of uric acid, or hyperuricemia, uric acid crystals can concentrate in the joints. However, many people with higher uric acid levels never get gout. Doctors place gout under the umbrella term “arthritis” — a broad range of joint diseases and joint pain.

People with chronic gout may develop tophaceous gout, which can affect any joint or soft tissue in the body, potentially leading to a reduced range of motion or even disability. However, treatments are available to alleviate the symptoms and minimize flares. Asparagus, beans, some other plant-based foods, and mushrooms are also sources of purines, but research suggests that these do not trigger gout attacks and do not impact uric acid levels. This stage is the period in between attacks of acute gout.

Acute Gouty Arthritis

Assuming no other medical problems that require an adjusted dose, for an attack of gout a patient would receive two tablets of colchicine, 0.6mg each, as soon as possible after a gout attack starts. They would then receive one additional tablet an hour later. Colchicine dose needs to be adjusted in patients with significantly decreased kidney function. Colchicine has interactions with certain other medications, most notably clarithromycin (Biaxin®). Whatever the mechanism of the elevated uric acid, the key event in gout is the movement of uric acid crystals into the joint fluid. This cycle also recruits more white blood cells to the joint, which accelerates the inflammatory process.

What is the main cause of gout?

Gout is caused by a condition known as hyperuricemia, where there is too much uric acid in the body. The body makes uric acid when it breaks down purines, which are found in your body and the foods you eat.

The high cost of this class of drugs may also discourage their use for treating gout. Febuxostat is only recommended in those who cannot tolerate allopurinol. There are concerns about more deaths with febuxostat compared to allopurinol. Febuxostat may also increase the rate of gout flares during early treatment. However, there is tentative evidence that febuxostat may bring down urate levels more than allopurinol.

It is often linked with obesity, high blood pressure, high levels of lipids in the blood , and diabetes. Tophi occur when the person’s blood uric acid level remains high over a long period of time. The uric acid deposits in joints and forms precipitates. These precipitates can become quite large, resulting in bumps and swellings on the joints.
