Chronic Tophaceous Gout

A doctor may recommend changes to your diet to reduce the risk of gout attacks returning. However, while diet is a risk factor for gout, it is not the sole cause. A low-purine diet should not be considered a cure for gout and dietary alterations should always be discussed with a doctor. The symptoms of gout may be sudden and often start at night. The affected joint swells and becomes red, hot, and painful. The joint may become stiff and hurt more when you touch it.

What Gout Affects

Cherry juice, which has long been an alternative remedy and which had anecdotal support, now has been studied. At the American College of Rheumatology meeting in November there were two studies looking at cherry juice. It appears that cherry juice may have a small effect in decreasing production of uric acid. It also, possibly through its Vitamin C content, can increase the excretion of uric acid by the kidney. This involves having a pharmacist put together a solution of allopurinol of very low and then gradually increasing concentrations over the course of a month.

There is a rare effect on the nerves and muscles with long-term use of colchicine, and a blood test from the muscle is monitored at approximately six-month intervals in patients taking colchicine on a regular basis. There are medications that block the production of uric acid and others that help your body remove uric acid, such as allopurinol and probenecid, respectively. Tophi can be aspirated or the tophaceous material expressed and examined under polarize microscopy as well to confirm a diagnosis of chronic tophaceous gout. About 2 o’clock in the morning, he is awakened by a severe pain in the great toe; more rarely in the heel, ankle or instep. Dietary causes account for about 12% of gout, and include a strong association with the consumption of alcohol, sugar-sweetened beverages, meat, and seafood. Among foods richest in purines yielding high amounts of uric acid are dried anchovies, shrimp, organ meat, dried mushrooms, seaweed, and beer yeast.

In the first six months of using allopurinol, gout attacks can occur more frequently. Understanding this, and having anti-inflammatories on hand, is important. One of the most popular is that gout only affects the big toe.

Consider initiating urate-lowering therapy in select patients (see “Indications” in “Urate-lowering therapy” for details). There is a lack of consensus regarding strict diagnostic criteria for gout in clinical settings.
