The first metatarso-phalangeal joint is the single most likely joint to be affected by gout, but many other joints can be involved. No formal studies have addressed the potential for cross-allergenicity of allopurinol and febuxostat, but the chemical structure of the two agents is quite different. When And Why Tophi Develop

In about 70% of acute gout attacks, a single joint becomes suddenly inflamed, often with excruciating pain, erythema, swelling and exquisite tenderness. For the patient of this case report presenting with CTS, it was particularly exceptional that he had no visible tophi in any part of the body. To our limited knowledge, the literature has yet to report tophi-associated chronic CTS in a patient who lacks visible signs of tophi but has a family medical history of gout. Here, we describe such a case of CTS without significantly visible tophi in a 25-year-old male. Untreated gout may cause deposits of urate crystals to form under the skin in nodules called tophi (TOE-fie). Tophi can develop in several areas, such as your fingers, hands, feet, elbows or Achilles tendons along the backs of your ankles. Does soaking in hot water help gout?

Tophus Treatment

Performance of surgical correction has been planned in an orthopedic department. Surgical treatment is seldom required for gout and is usually reserved for cases of recurrent attacks with deformities, severe pain and joint destruction . The main indication for surgery in patients with tophaceous gout is sepsis or infection of ulcerated tophi, followed by mechanical problems, confirmation of diagnosis and pain control . A hydrosurgery system applying a highly pressurized saline stream has also been used with good results for the debridement of tophi . In the early stages, surgical arthroplasty can be carried out, but simple enucleation of the tophi may lead to complications such as skin necrosis, tendon and joint exposures .
