Calcium pyrophosphate deposition disease may mimic polymyalgia rheumatica. Pegloticase is a recombinant pegylated form of uricase, the enzyme that catalyzes conversion of urate to allantoin. It has been shown effective in treating chronic gout in adult patients refractory to conventional therapy.

Tophi form in the joints, cartilage, bones, and other places throughout the body. Sometimes, tophi break through the skin and appear as white or yellowish-white, chalky nodules. Without treatment, tophi may develop on average about ten years after the onset of gout, although their first appearance can range from three to forty-two years. The development of gouty tophi can also limit joint function and cause bone destruction, leading to noticeable disabilities, especially when gout cannot successfully be treated. They are more apt to appear early in the course of the disease in people who are older. Any urate lowering therapy should not be started during an acute attack, since the dissolution of crystal deposits increases the risk of gout attacks.

Multiarticular Chronic Tophaceous Gout With Severe And Multiple Ulcerations: A Case Report

Gout is a common metabolic disease caused by deposition of MSU crystals in peripheral joints. Tophi are formed by nodular accumulation of MSU crystals in soft tissue that usually appears in the metatarsophalangeal joint or knee. Tophi are usually present in patients who have had gouty arthritis for at least 10 years. Microscopically, tophi are chronic foreign body granuloma-like structures containing collections of MSU crystals surrounded by inflammatory cells and connective tissue. Surgical removal of tophi is also reported to reduce pain, maximize function, and improve cosmesis.

In some instances, it might be difficult to excise all the tophaceous material especially if they had infiltrated into the entire segment of the tendon or were tightly adherent to the neurovascular bundles. In such circumstances, we first identified the tendon or the neurovascular bundles in an unaffected area and traced them into the affected zone. Then, we removed the tophaceous material preserving as much as possible the now clearly identified tendon and the neurovascular bundles. For areas where the joint was involved, the joint capsule and collateral ligaments were usually damaged by the infiltrating tophaceous material. This could be addressed after the debridement by making use of the remaining soft tissue to create a pseudo-joint capsule in order to place the joint in an acceptable alignment. Kirchner wire pinning might then be used to further stabilize the joint.

A Rare Case Of Ulcerated Tophaceous Gout

A single dose of triamcinolone acetonide is effective in most patients. However, patients with longstanding or polyarticular attacks may require repeat doses. Oral prednisone is an option when repeat dosing is anticipated. A regimen of 0.5 mg per kg of prednisone on day 1 and tapered by 5 mg each day is also very effective. In response to the public comments and input from the Technical Expert Panel for the review on diagnosis of gout, the testing of urinary uric acid was removed and “plain X-ray” was added. Several public and TEP comments noted that urinary uric acid is not commonly used and plain x-ray is sometimes used in primary care settings. Tophi can be deposited in various structures, including the flexor tendons, tendon sheaths, the carpal tunnel floor, transverse carpal ligament, and even the median nerve. In the current case, several flexor digitorum superficialis and flexor digitorum profundus tendons were affected by tophi, and more deep tophi were found adhering to the distal volar radius. Interestingly, in this patient the pronator quadratus muscle was also affected, which had never been reported in the literature. The day after surgery, the patient reported marked lessening of numbness in the thumb, index finger, middle finger, and half of the ring finger of his right hand. We then encouraged him to actively flex and extend his hand; uric acid-lowering medication was recommended. The patient was followed closely as an outpatient under orthopedic surgery services and rheumatology services.

Rarely, gout can produce spinal cord impingement when deposition in tissues produces a local mass. There is another case report in which ultrasound of the finger showed increased echogenicity and heterogeneity in the tendon. The patient was treated with oral non-steroidal anti-inflammatory medication without clinical improvement.

Surgery to remove tophi that are beginning to break through to the skin is sometimes needed and can prevent severe infectious problems and osteomyelitis. It can take 18 months or more for allopurinol or febuxostat to lead to tophus resorption, and perhaps 3 months with pegloticase. Some patients have tophi which need to be addressed with a more urgent timetable due to active or impending infection.

We report one such case of a 39-year-old male with poorly controlled polyarticular tophaceous gout presenting with a massive swelling of the left elbow simulating a soft tissue tumor. Subsequent investigations confirmed it to be a massive tophus which was then surgically excised, as the mass was not responding to the medical management.

There is a risk for an acute gouty flare-up when prophylaxis is discontinued. However, most patients are able to remain on urate-lowering agents alone. If patients do not tolerate daily doses of colchicine, a low daily dose of a selected NSAID can be used instead. 2 In untreated patients, chronic tophaceous gout may develop, which is characterized by chronic destructive polyarticular involvement and tophi. Tophi can occur in soft tissue, osseous tissues, ligaments and different organs and either in presence or in absence of gouty arthritis.

Tophi are typically found on the helix of the ears, on fingers, toes, wrists and knees, on the olecranon bursae, on the Achilles tendons and also rarely on the sclerae, subconjuctivally and on the cardiac valves. The majority of individuals with gout are initially seen, diagnosed, and treated in primary and urgent care settings. Therefore primary care physicians and emergency medicine physicians are well-positioned to diagnose early-stage gout and implement management strategies. In fact, evidence from a 2011 survey of rheumatologists suggests that SF analysis is underused in the rheumatology setting as well.9 Ultrasound and dual-energy computed tomography are just beginning to be used to diagnose gout in some settings. Radiographic scans of the patient's hands revealed bilateral juxta-articular osteopenia involving the metacarpophalangeal regions and bilateral symmetric radial and ulnar carpal joint space loss with associated erosive changes consistent with RA . The first metacarpophalangeal joint in the patient's left hand was subluxated and loss of the ulnar styloid was visible.

Why do people get gout?

Colchicine can be used orally to treat gout, and this treatment is often effective, although patients commonly develop diarrhea before attacks are resolved. With the new FDA indication came a new dosage recommendation, from the study of Terkeltaub et al. The recommended colchicine dosage is 2 tabs (0.6mg each) followed by one tab (0.6mg) an hour later, with no further colchicine.

Alcohol Use – Alcohol use is highly associated with gout in younger adults. It appears to play less of a role among elderly patients, especially among women with gout. We preferred not to excise skin until the very end of surgical excision. We observed that after excision of abundant tophaceous material we usually had excessive skin left behind as a result of the chronic stretching of skin by the tophi. Hence, our approach was to take down the tourniquet and then observe the circulation of the skin flap.

We prefer the surgical incision to be longitudinal and made over the most affected area.
