The human body makes uric acid during the breakdown of chemicals called purines found in certain food and drinks. This normal byproduct goes through the kidneys and exits the body when you pee. Defective uric acid excretion also occurs when the kidneys are functioning poorly. This stage is marked by acute gout attacks causing pain and inflammation in one or more joints. Although alcohol can bring on gout attacks, genetics are much more important than alcohol in defining who gets gout, and many who never drink alcohol suffer from gout. This situation has been mimicked in more recent times when imbibers of “moonshine whiskey,” often made in radiators containing lead, developed a lead poisoning-associated gout (“Saturnine gout”).

For severe cases of gout that don’t respond to the treatments above, your physician may recommend corticosteroid injections into the affected joint. Injections can also be useful for people who are sensitive to oral medication. If other members of your family have had gout, you're more likely to develop the disease. After the most severe pain subsides, some joint discomfort may last from a few days to a few weeks. Later attacks are likely to last longer and affect more joints.

This typically occurs when there are high levels of uric acid in the blood. Uric acid is produced when purine-rich foods such as steak, organ meats, seafood, and alcoholic beverages, especially beer and drinks sweetened with fruit sugar , are consumed in a large quantity. Normally, uric acid dissolves in the blood and gets excreted into the urine. However, sometimes there is too much uric acid or too little uric acid excreted causing urate crystals in the joint. If your symptoms do not improve with initial treatments, your doctor may recommend a synovial fluid analysis.

The pain may go away on its own over a few hours, or it may take a few weeks. The first attack of gouty arthritis usually happens in just one joint. After an initial attack has subsided, there are several types of medications that that work to reduce uric acid levels in the body, prevent gout attacks, and prevent long-term joint damage from gout. These medications work either by increasing the excretion of uric acid in the urine, or by preventing uric acid production. These include probenecid (Benemid®), sulfinpyrazone (Anturane®), and allopurinol (Zyloprim®).

If we can prevent antibody formation, it has been shown that infusion reactions are dramatically decreased, and the effectiveness of pegloticase is also much better maintained. A larger trial with methotrexate and pegloticase is in progress. Along with diet, physical activity can help with weight loss, and gout has been associated with being overweight. There are many circumstances where, however ideal it would be, no fluid or other specimen is available to examine, but a diagnosis of gout needs to be made.
