Pseudogout And Its Treatment

It also can cause a more lasting arthritis that mimics osteoarthritis or rheumatoid arthritis.The condition most often involves the knees, but can affect wrists, shoulders, ankles, elbows, hands or other joints. Gout, a common type of crystalline arthropathy, results from a buildup of urate crystals in a joint, causing pain and inflammation. Urate crystals can form in people who have high levels of uric acid in the blood. This can happen when the body produces too much uric acid or the kidneys cannot remove enough. In contrast to gouty arthritis, no therapy is available to prevent the deposition of calcium pyrophosphate crystals or to remove the calcium pyrophosphate deposits already present. Patients with chronic arthropathy from CPPD in the form of pseudo-osteoarthritis or pseudo-rheumatoid pattern are managed with physical therapy, analgesics or NSAIDs.

Totally clear, almost colorless, viscous fluids may still contain MSU or CPPD crystals but suggest that there is little or no inflammatory response to the crystals. During acute attacks of gout or pseudogout effusions are cloudy because of the leukocyte response. Microscopic examination of a wet drop, differentials on a stained smear and leukocyte counts are commonly used to further characterize the response. Evidence for the presence of crystals can begin to accumulate with the initial gross examination of the fluid aspirated .

Arthritis And Pseudogout

Gout is a disease resulting from the deposition of urate crystals caused by the overproduction or underexcretion of uric acid. The disease is often, but not always, associated with elevated serum uric acid levels. Clinical manifestations include acute and chronic arthritis, tophi, interstitial renal disease and uric acid nephrolithiasis. The diagnosis is based on the identification of uric acid crystals in joints, tissues or body fluids. Treatment goals include termination of the acute attack, prevention of recurrent attacks and prevention of complications associated with the deposition of urate crystals in tissues. Acute attacks may be terminated with the use of nonsteroidal anti-inflammatory agents, colchicine or intra-articular injections of corticosteroids.

Acute episodes can last for days to weeks, and symptoms may subside without treatment. Calcium pyrophosphate crystal deposits may accumulate around the bones of the spine and cause back or neck pain and/or loss of mobility. Many patients with CPDD develop chronic arthritis which can resemble osteoarthritis. There is one other possible cause of CPPD crystal deposition disease--heredity/genetic makeup. In some cases, CPPD crystal deposition disease may run in families and may be more common in some ethnic groups.

What Is Pseudogout?

The steroids reduce inflammation but can cause serious complications if used long-term. The imaging can reveal calcification in the knee or wrist, which suggests pseudogout, says Dr. Hughes. “It affects joints that are typically not affected in garden-variety osteoarthritis,” says Dr. Davis. Most commonly, pseudogout appears in the knees and sometimes the wrist, though it can also affect the hips, ankles, and other joints. Since CPPD are often only weakly or virtually non-birefringent they may actually be best seen on this regular light examination. Phase contrast or adjusting the condenser may produce a phase effect and can better delineate the CPPD crystals. Especially when there are few cells, crystals may be best seen in clumps of tissue fibrils or tissue fragments. Pseudogout - Pseudogout is the acute form of CPPD, which is due to intercurrent conditions. The symptoms resemble those of gout; however, they tend to be less debilitating.

The molecular structure of calcium pyrophosphate has the potential of triggering inflammatory responses. The presence of chondrocalcinosis has associations with the degradation of menisci and synovial tissue. Patients with underlying joint comorbidities, such as osteoarthritis, have an increased risk for acute CPP arthritis. When deposited, CPP crystals activate the immune system, promoting inflammation and fibrocartilage injury. Acute calcium pyrophosphate arthritis is generally self-limited, and the inflammation usually resolves within days to weeks of treatment.

Inflammatory Arthritis

If the rash was relatively mild, one option is an oral desensitization regimen for that agent. Along with diet, physical activity can help with weight loss, and gout has been associated with being overweight. Treatments used for prevention, such as allopurinol can actually make things worse if given during an attack, and so need to be held back until the attack has resolved for several weeks. Collections of uric acid need to be searched for, and they can be in numerous locations (see Figures 7-10).

Can Pseudogout affect the neck?

First, pseudogout attacks of the cervical yellow ligament can cause acute neck pain. Second, calcification of the cervical yellow ligament raises suspicion for pseudogout. There have been some reports of cervical canal stenosis caused by CPPD crystal deposition [1].

COL is most preferably given through an oral route because it is highly caustic when administered intravenously and its extravasation can cause irritation and tissue necrosis. Nevertheless, in case of severe gastritis where NSAIDs and oral colchicine should be avoided, the use of intravenous colchicine pre-diluted with 0.9% NaCl has been reported to be equally efficacious . The hydrolysis of adenosine triphosphate generates energy and yields a compound called inorganic pyrophosphate . The degradation of PPi is catalyzed by inorganic pyrophosphatase in the fibroblasts of chondrocytes.

Features Of Cppd:

While it is reasonable to decrease or avoid these foods, it has been found that a high purine-rich diet does not increase the risk of gout, or aggrevate symptoms in research studies. The symptoms of pseudogout are very similar to those of gout, although thr flare-ups are usually less severe. It takes a long time without treatment to reach the stage of chronic tophaceous gout – around 10 years.
