Easing The Pain Of Acute Gout

I dare not move because of the pain, and yet I have to make repeated frantic visits to the toilet. It seems to be assumed, even among doctors, that gout is an extinct Victorian ailment due to too much port and cigars. For over 10 years at various general practitioner and hospital visits, I was repeatedly misdiagnosed as having a sprained ankle or a broken bone in my foot. I would experience extreme pain for a couple of days which would then ease off over the course of the following week.

In total, 394 patients were diagnosed at least once with an ICD-10 code indicating gout during the study period (2009–2013), and their medical records were reviewed. Between 2009 and 2013 in Olskroken and Masthugget, 173 and 89 patients in total were diagnosed with gout at least once, respectively. Early data has looked at medications such as methotrexate, mycophenolate mofetil (Cellcept®) and azathioprine (Imura®) given during the course of pegloticase treatement, and the results to date have been very encouraging. A larger trial with methotrexate and pegloticase is in progress. If an attack of gout is allowed to last more than a day or so before treatment is started, the response to treatment may be much slower. Late in gout, if untreated, multiple joints can be involved, including the fingers and wrists. The shoulder joint is very rarely involved by gout and the same is true of the hip.

Since it is hard to heal the skin after a tophus is removed, a skin graft may be needed. For this reason, we often try hard to manage the tophus medically. If we give high doses of medication to lower the urate level, such as allopurinol, over time the tophus will gradually reabsorb. In severe cases, we may consider using the intravenous medication pegloticase (Krystexxa®), since it lowers the urate level the most dramatically, and can lead to the fastest shrinkage of the tophus. Diet has been discussed in more detail above, and gout is clearly one of the rheumatic diseases where diet is unequivocally important.

Of these preparations, betamethasone lasts the shortest time in the joint of these preparations, but gout tends to be self-limited within a few weeks, in any case, so this option can be quite successful. To warrant current conclusions, well-designed studies are still needed.
