The oral form of colchicine has been available for gout treatment for 70 years. However there has been no FDA-approved preparation until recently and the branded product is being marketed under the name of Colcrys®; URL Pharma, Inc.). Traditional radiographs are largely unhelpful in the diagnosis of gout since the crystals are relatively radiolucent, and bony changes do not occur until late in the course of the disease.
Dosage requirements in excess of 300 mg should be administered in divided doses. The minimal effective dosage is 100 to 200 mg daily and the maximal recommended dosage is 800 mg daily. This class of drugs works quickly to relieve pain and is a cost-effective option. The data do not suggest, however, that these medications are better than any other treatment option for gout attacks. Indomethacin, a prescription medicine, is commonly used; however, other anti-inflammatory medications may be used, such as ibuprofen and naproxen , and sulindac, by prescription.
Corticosteroids are often used to treat severe allopurinol hypersensitivity syndrome. Allopurinol blocks xanthine oxidase and thus reduces the generation of uric acid. Corticosteroids can be given to patients with gout who cannot use NSAIDs or colchicine. Steroids can be given orally, IV, intramuscularly , or intra-articularly.
