Uric acid crystals can be thought of like matches, which can sit quietly or can be ignited. Crystals can be present for years in the cartilage, or even in the joint fluid, without causing inflammation. Then, at some point, due to increasing number of crystals or other inciting factor, the matches are “struck” and the inflammation begins.

Left untreated, a gout attack will usually resolve itself within a few days or weeks. Chronic gout can permanently damage a joint’s tissues and decrease its range of motion. For this reason, it is important to recognize symptoms, understand risk factors, get an accurate diagnosis, and treat and prevent gout. Untreated gout may cause deposits of urate crystals to form under the skin in nodules called tophi (TOE-fie). Tophi can develop in several areas, such as your fingers, hands, feet, elbows or Achilles tendons along the backs of your ankles. Tophi usually aren't painful, but they can become swollen and tender during gout attacks.

Are There Home Remedies For Gout?

These tophi tend to happen in the areas of the body that are coldest, including the ears, fingertips and elbows. Uric acid crystals tend to settle in the big toe, as in podagra. This joint is where there tends to be increased impact from walking and supporting the weight of the body. Uric acid crystals are also less soluble under acidic conditions and in cooler body parts further away from the torso, such as are found in the big toe and other joints of the hands and feet. Unlike women, men with gout were not at greater risk for angina, transient ischemic attack, or stroke. Vaccination has been associated with increased risk of gout flares.

Is Gout Painful?

Rates of gout approximately doubled between 1990 and 2010. This rise is believed to be due to increasing life expectancy, changes in diet and an increase in diseases associated with gout, such as metabolic syndrome and high blood pressure. Factors that influence rates of gout include age, race, and the season of the year. While historically it is not recommended to start allopurinol during an acute attack of gout, this practice appears acceptable.

Other conditions which can mimic gout, should be definitively ruled out through crystal identification in joint fluid whenever possible. Anakinra (brand name Kineret®) is a biologic medication that blocks the inflammatory protein IL1. This medication is injected subcutaneously by the patient once a day, usually for 3 days, but can be used longer if needed to resolve a flare. Although much data supports the effectiveness and safety of this medication for gout, it is expensive and not as yet FDA approved for gout flares.
