A 2011 survey in the United States indicated that 3.9% of the population had gout, whereas 21.4% had hyperuricemia without having symptoms. In general, the water solubility of uric acid and its alkali metal and alkaline earth salts is rather low. All these salts exhibit greater solubility in hot water than cold, allowing for easy recrystallization. The solubility of the acid and its salts in ethanol is very low or negligible. In ethanol/water mixtures, the solubilities are somewhere between the end values for pure ethanol and pure water.

This is a result of the FDA effort to review and standardize the production of drugs which have been around a long time and were not previously reviewed by FDA. Prophylactic medications are used during approximately the first six months of therapy with a medication to lower high levels of uric acid to either prevent gout flares or decrease the number and severity of flares. This is because any medication or intervention that either increases or decreases the uric acid level in the bloodstream can trigger a gout attack.

In 1882, the Ukrainian chemist Ivan Horbaczewski first synthesized uric acid by melting urea with glycine. Drug options to reduce uric acid levels are primarily reserved for those with full-blown gout—leaving hyperuricemic individuals without easy options. The fruit of the Terminalia bellerica tree has been used in Ayurvedic medicine for many years, mainly for removal of kidney stones and decreasing inflammation. The extract being studied is derived from this edible tree fruit that contains 15% tannins. A tree fruit extract has been studied for its ability to inhibit uric acid production in humans.8 An extract of the Terminalia bellerica fruit is rapidly emerging as a natural uric acid lowering nutrient.

Saturation levels of uric acid in blood may result in one form of kidney stones when the urate crystallizes in the kidney. These uric acid stones are radiolucent and so do not appear on an abdominal plain X-ray. Uric acid crystals can also promote the formation of calcium oxalate stones, acting as "seed crystals".

Cancer Treatment

People with gout suffer from joint pain, most often in their toes, but in other joints as well. The test is also ordered when cancer patients undergoing chemotherapy or radiation therapy to ensure that uric acid levels do not get dangerously high. Uric acid crystals can form stones in the kidneys, in the ureters or in the bladder itself. For example, the deposits may be caused by not drinking enough liquids.

This is because the blood and joint levels of uric acid reach a certain level, called a “steady state,” at a given level of blood uric acid.
