People who have a thyroid condition, kidney failure, or disorders that affect calcium, phosphate, or iron metabolism are at increased risk for pseudogout. It isn't clear why crystals form in your joints and cause pseudogout, but the risk increases with age. Treatments can help relieve pain and reduce inflammation. It's diagnosed by the study of the synovial fluid from the inflamed joint, which is observed under a microscope for CPPD crystals.

In patients presenting with acute joint swelling, the differential diagnosis should include evaluation for infection. Because CPPD disease is associated with variety of underlying conditions, practitioners should screen for hyperparathyroidism, hypothyroidism, hypomagnesemia, hypophosphatasia, and hemochromatosis. Blood should be tested for intact parathyroid hormone, calcium, phosphorous, thyroid-stimulating hormone, magnesium, ferritin, iron transferrin, and alkaline phosphatase.

It's not clear why some people have symptoms and others don't. The diagnosis of CPPD can be suspected by certain X-rays and imaging studies or computed tomography , but the findings of CPPD crystals on synovial fluid analysis leads to a more definite diagnosis. Although the disease processes of gout and rheumatoid arthritis are quite different, both are characterized by redness, swelling, and pain in the joints that can cause serious disability. Colchicine offers the best response when initiated within 48 hours of acute gout onset. Patients usually notice improvement within 24 to 48 hours of initiating therapy. During acute gout, oral colchicine can be started at 0.6 mg three or four times daily for 2 days, then decreased to twice daily.

Fluid is removed through a needle from the inflamed joint in a procedure called "arthrocentesis." Removing the fluid also may help reduce the pressure within the joint and thereby reduce pain. The risk of pseudogout is higher for people who have excessive calcium or iron in their blood or too little magnesium. Not all patients will develop side effects from these medicines. How often any side effect occurs varies from patient to patient.
