For the combination of these outcomes, the two medications were the same. There were some problems with interpreting the study, since almost all the patients who died had already stopped their gout medication, whether allopurinol or febuxostat. Many rheumatologists do not think this is a definitive study, and there is other data that does not show increased heart risk with febuxostat. However, the FDA has interpreted this study and put a warning on febuxostat that it should be used second line, after allopurinol. For those with a higher level, for example, 10.0 mg/dL, diet alone will not usually prevent gout. For the latter, even a very strict diet only reduces the blood uric acid by about 1 mg/dL- not enough, in general, to keep uric acid from precipitating in the joints.

This procedure involves removing the painful joint and replacing it with artificial parts. The goal of joint replacement is to provide pain relief, as well as to maintain joint movement. The knee is the most common joint requiring replacement due to gout. Once gout progresses to the chronic stage — which takes several years — joints may have permanent damage and deformity, and pain may be persistent. When properly treated, most cases of gout will not progress to this disabling stage. Fortunately, gout attacks can be controlled and treated with medication.

The FAST trial challenges this, and it may well be that they are equally protective. One potential advantage of febuxostat is that it is structurally quite different from allopurinol, and therefore likely can be used in patients who are allergic to allopurinol. Only a limited number of patients who were allergic to allopurinol have been studied to date, but the drug was tolerated in those patients. Another advantage is that its excretion is handled more by the liver than the kidney, unlike allopurinol, and febuxostat may thus have some advantage in patients with kidney dysfunction. Late in gout, if untreated, multiple joints can be involved, including the fingers and wrists. The shoulder joint is very rarely involved by gout and the same is true of the hip.

Causes And Symptoms

The main symptom of gout is waking up in the middle of the night with an acute throbbing pain in the big toe, which is swollen. The pain lasts for around three or four hours and then subsides. However, pain in the same toe usually returns within a few months. Juvenile arthritisis the term used to describe arthritis when it begins before age 16.

Untreated gout may cause deposits of urate crystals to form under the skin in nodules called tophi (TOE-fie). Tophi can develop in several areas, such as your fingers, hands, feet, elbows or Achilles tendons along the backs of your ankles.
