The most feared adverse reaction is hypersensitivity syndrome associated with fever, bone marrow suppression, hepatic toxicity, renal failure and a systemic hypersensitivity vasculitis.32 Fortunately, this life-threatening syndrome is rare. In patients with a creatinine clearance of 100 mL per minute (1.67 mL per s) or greater, the initial dosage can be up to 300 mg, with a maximum dosage of 600 mg. In patients with a creatinine clearance of 60 mL per minute (1.00 mL per s), the maximum dosage of allopurinol is 200 mg per day. The maximum dosage in patients with a creatinine clearance of 30 mL per minute (0.50 mL per s) is 100 mg per day, and 100 mg every two to three days in patients with a creatinine clearance of 10 mL per minute (0.16 mL per s) or less.

Because exposure to drugs is a common cause of granulomatous liver disease, a history of medication use is essential. More-extensive evaluation, such as abdominal or chest CT scanning, may be necessary if lymphoma is suspected. Granulomatous liver diseases are more accurately classified as infiltrative diseases but are discussed here because the pattern of liver enzyme abnormality resembles that seen with cholestasis. The finding of granulomas on liver biopsy is often expected, for example, in patients with suspected primary biliary cirrhosis who present with cholestasis and a positive mitochondrial antibody or patients with known sarcoidosis who present with cholestasis. On the other hand, when granulomas are found on liver biopsy unexpectedly or as part of an evaluation for a systemic illness, a thorough investigation should be undertaken to look for the underlying cause.

They should be encouraged to eat low-fat or nonfat dairy products, and vegetables; evidence suggests that increased vegetable intake may reduce serum urate levels and decrease risk factors for urolithiasis. Acute gouty arthritis most commonly begins with involvement of a single joint or multiple joints in the lower extremities, most commonly the first metatarsophalangeal (i.e., podagra), midtarsal, ankle, or knee joints. Pain, erythema, and swelling often begin in the early morning and increase and peak within 24 to 48 hours. The pain is severe, and patients often cannot wear socks or touch bedsheets during flare-ups.
