My personal feeling is that herbal and other alternative/complementary approaches may well ultimately be proven to improve inflammation in gout, but lowering uric acid is the key to success in gout management. The main message of this review is to emphasize how dramatically effective standard medication is for gout, both in acute treatment and prevention. Many of my patients have explored a variety of non-traditional approaches to gout, often in combination with traditional measures. Like Uloric®, Krystexxa® does not appear dependent on the kidney to be removed from the body, allowing it to be considered in patients with decreased kidney function. Because Krystexxa® is given intravenously, it would be expected that the great majority of its use would be by rheumatologists rather than by internists or primary care physicians.
You probably won't know that you have an elevated uric acid level in your blood until you have had your first gout attack. In order to assess the best treatment in the normouricemic and refractory attack, it is necessary to consider the pathophysiology of gout. The initial phase is the formation of MSU crystals in a synovial fluid supersaturated with MSU . Next step is the amplification phase, in which there is activation of adhesion proteins and leukocyte recruitment to the synovial tissue by IL-1, opsonization of MSU crystals by IgG and complement. This process facilitates MSU phagocytosis by monocytes and neutrophils, resulting in tissue damage .

A prescription medicine called colchicine helps reduce pain, swelling, and inflammation. Gout may occur after taking medicines that interfere with the removal of uric acid from the body. People who take certain medicines, such as hydrochlorothiazide and other water pills, may have a higher level of uric acid in the blood. Your doctor will review your medical history for any contraindications for gout medications. You may be directed to stop taking diuretics , as these can contribute to uric acid buildup. In addition, the disease overall may progress with “recurrent or more frequent gout attacks with longer duration, the involvement of more joints, and the presence of tophi,” Dr. Meysami says.

What Is The Prognosis For Gout?
As time goes on, gout attacks may also become more frequent. High uric acid levels may cause kidney stones and, sometimes, damage the kidneys. About 15 people out of 100 with gout develop kidney stones.
Here, there was no difference in death rates in patients on febuxostat compared to those on allopurinol. This trial actually had many fewer dropouts in the study and overall reviewers have felt that the FAST trial is a more solid base than the CARES trial on which to base decisions about the use of febuxostat. Some have called for the FDA to reconsider its recommendations, but no changes made to date. The FAST trial gives considerable comfort to those patients presently on febuxostat. It is generally agreed that we have no evidence that febuxostat is a negative for the heart, just the question of the CARES trial as to whether it is not as protective as allopurinol. The FAST trial challenges this, and it may well be that they are equally protective.
A joint infection must be excluded, however, as glucocorticoids worsen this condition. Hyperuricemia and gout are associated with an increased overall likelihood of mortality. Whether this is directly attributable to hyperuricemia or gout or to gout-associated diseases has been much debated.
The goals of treatment are to relieve pain, shorten the duration of inflammation during an acute attack, prevent future attacks and prevent joint damage. Left untreated, a gout attack will usually resolve itself within a few days or weeks. Chronic gout can permanently damage a joint’s tissues and decrease its range of motion.
Prevention
Your health care professional will want to know when the pain started, what aggravates or eases it, and if any other joints in your body are involved. They may also ask you about your diet, alcohol consumption, or whether or not you have a family history of gout or any other related medical conditions. The synovial fluid from the affected joint will need to be examined in order to identify the needle-like crystals that are part of gout. To get a sample of the synovial fluid, a doctor performs an arthrocentesis. But the most important fork in the road for gout sufferers is whether to start taking a drug that will lower their uric acid levels.

In the United States in 2015–16, the overall prevalence of gout in adults was 3.9%, corresponding to a total affected population of 9.2 million.
