How Should You Eat If You Have Diabetes And Gout?

Diabetes also is a well-known risk factor for cardiovascular disease. Despite being known as a cardiovascular risk factor, it is not known whether gout is a cardiovascular risk equivalent similar to diabetes. As for controlling blood sugar, regular exercise, weight loss, a low-glycemic/low-sugar diet, and a solid supplement regimen protect against all aspects of metabolic syndrome. Work on controlling these risk factors, and you can dramatically reduce your risk of gout. Diabetic retinopathy is a specific fundus lesion that is the main cause of blindness in patients with diabetes .

Outwardly it would seem true, but we recently discovered that glucose may act to induce obesity and insulin resistance by being converted to fructose in the liver . Specifically, high concentrations of glucose, such as occurs in soft drinks, can induce the activation of the polyol pathway in the liver, resulting in the generation of fructose. In turn, the fructose is then metabolized by KHK, resulting in fructose-dependent effects. Indeed, glucose-induced weight gain, fat accumulation, fatty liver, and insulin resistance are all dependent on KHK. While some visceral fat and weight gain occur in glucose-fed mice lacking KHK, the development of fatty liver and hyperinsulinemia are almost entirely dependent on glucose-induced fructose metabolism .

Treating Patients At Risk For Or Diagnosed With Type 2 Diabetes Should Be Closely Monitored For Gout

Today, fructose intake has been shown to induce all features of metabolic syndrome in rats, as well as oxidative stress, endothelial dysfunction, fatty liver, microalbuminuria and kidney disease (rev. in 1). Similar findings can be shown when animals are fed sucrose or high-fructose corn syrup , both which contain fructose . In contrast, administration of glucose or starch results in fewer features of metabolic syndrome when provided equivalent intake . The intake of added sugars, such as from table sugar and high-fructose corn syrup has increased dramatically in the last hundred years and correlates closely with the rise in obesity, metabolic syndrome, and diabetes. Fructose is a major component of added sugars and is distinct from other sugars in its ability to cause intracellular ATP depletion, nucleotide turnover, and the generation of uric acid.

Gout Treatment May Help Prevent Obesity

Cherries are also high in potassium and contribute to helping the body maintain a slightly alkaline state, which prevents the formation of acids in the blood, especially uric acid. In severe and long-term cases of gout, the progressive accumulation of urate crystals in the joints can causejoint erosion as well as bone and joint deformities. Gout is caused by the accumulation of uric acid crystals, a waste product that’s formed from the breakdown of purines — substances found in foods such as organ meats, asparagus, anchovies, herring, and mushrooms. Gout is a type of arthritis characterized by sudden, intense pain, redness, heat, swelling, and tenderness in the joints.

All studies were based on large databases, and the diagnosis of DM or gout was mainly based on diagnostic codes, self-report, prescriptions for drug use, or laboratory findings. The quality of the included studies was evaluated according to NOS, and the NOS scores ranged from 6 to 8. The detailed characteristics of the included studies are illustrated in Tables 1 and 2. "Furthermore, the link between hyperuricemia and the risk of type 2 diabetes may originate at the renal level, as insulin resistance and higher insulin levels are known to reduce renal excretion of urate," he and his colleagues wrote.

Combination Of Gout And Diabetes Carries Increased Risk Of Amputation

Eat high-fiber foods, like pineapple, oats, cucumber, and carrots, and foods rich in omega-3 fatty acids, like sardines, salmon, and tofu. Avoid purine-rich foods, like dried beans, canned goods, instant noodles, and alcohol, since these can increase insulin resistance. You should also avoid foods high in fructose, like apples, bananas, chocolate, and soda, which raise your uric acid levels. Try to eat at the same times each day, and don’t skip any meals. This will keep your blood sugar levels consistent throughout the day. To the best of our knowledge, our study is the first to investigate the bidirectional association between gout and diabetes in Chinese population.

The prevalence of gout accounts for approximately 5% of the middle-aged and elderly global population, and the incidence of gout has increased steadily in recent years . Individuals with T2DM generally have a higher prevalence of high blood pressure , obesity, and decreased kidney function . Both DM and gout are related to a high risk of cardiovascular events, kidney failure, and mortality [4–7]. Therefore, the relationship between DM and gout has attracted great attention. Experimental studies from the 1950s showed the peculiar ability of fructose to induce insulin resistance in laboratory rats.

Beer and distilled liquors are associated with an increased risk of gout and recurring attacks. Moderate consumption of wine doesn't appear to increase the risk of gout attacks. Avoid alcohol during gout attacks, and limit alcohol, especially beer, between attacks.

How can gout be prevented?

Strategies for Preventing a Gout Attack 1. Avoid or limit alcohol.
2. Drink plenty of water.
3. Lose excess weight or maintain a healthy weight.
4. Treat sleep apnea.
5. Avoid foods that can trigger a gout flare-up.
6. Avoid medications that trigger gout.
7. Take prescription gout medication.
8. Foods that may trigger a gout attack.

Purines are nitrogen-containing compounds that are produced inside the body or can be found in certain foods and drinks.Gout occurs when urate crystals accumulate in the joints, causing intense pain and inflammation. Urate crystals can form when a person has high levels of uric acid in the blood. But the National Institutes of Health estimates that 6 million U.S. adults have had gout at some point in their lives, many with uncontrolled uric acid levels. That translates to tens of thousands of people at risk of diabetes and kidney disease, Matteson says. But new studies show that high uric acid levels in the blood are associated with a nearly 20% increased risk of developing diabetes and a more than 40% increased risk of developing kidney disease.

The ability for fructose to stimulate food intake and to lower metabolism provides a mechanism for how a high fructose intake may encourage weight gain and visceral fat accumulation. However, fructose or sucrose also alters fat stores and metabolism independent of excessive energy intake. Although weight gain is largely controlled by overall energy intake, other features of metabolic syndrome can occur independent of weight gain. For example, rats fed fructose develop fatty liver, hypertriglyceridemia, and insulin resistance when compared with rats fed isocaloric glucose or starch-enriched diets . Indeed, hypertriglyceridemia, fatty liver, and type 2 diabetes can be induced in metabolic syndrome–prone rats with caloric restriction provided the diet is high (40%) in sucrose . A recent epidemiological analysis in humans also found an association of diabetes prevalence with sugar availability that was independent of total energy intake .

Having one condition elevates the risk of developing the other. The possibility of a connection was affirmed in the late 18th century but again it doesn’t take a rocket scientist to come to this conclusion. This is obviously due to the fact that gout is linked with obesity, hypertension and little exercise or high blood pressure. Choi speculates that ongoing, low-level inflammation from gout may increase the risk for diabetes.
