Pseudogout has been recognized as having an underlying genetic component; however, comorbid conditions and environmental factors are thought to play a much stronger role. Some disorders that can lead to secondary pseudogout, such as hemochromatosis, do have a clear genetic cause.
Identification of CPP crystals on synovial fluid analysis or on imaging in a patient with typical symptoms confirms the diagnosis. Treatment is primarily symptomatic and consists of antiinflammatory medications . There is currently no specific treatment targeted at CPP crystals. Gout is an inflammatory crystal arthropathy caused by the precipitation and deposition of uric acid crystals in synovial fluid and tissues. Decreased renal excretion and/or increased production of uric acid leads to hyperuricemia, which is commonly asymptomatic but also predisposes to gout. Acute gout flares typically manifest with a severely painful big toe and occur most often in men following triggers such as alcohol consumption.

The clinical presentation resembles gout in its acute attacks of crystal synovitis and, thus, was previously called pseudogout. Acute CPPD arthritis is now the preferred term for this disease. Febuxostat is well tolerated without the need for dosage adjustment even in patients with mild renal impairment. The most frequently reported adverse effects are diarrhea, back pain, headaches, and arthralgias.
Rheumatologists are actively researching the causes of CPPD to better prevent and treat this form of arthritis. Because people with CPPD tend to be older and more prone to side effects from anti-inflammatory medications, they may benefit from seeing a rheumatologist, who can offer expertise in using such drugs and other therapies.
