Treatment for chronic gout is primarily focused on reducing the intake of purines in the diet, reducing the formation of uric acid from proteins, and/or helping your body to excrete more uric acid. Acute gout attacks are treated with anti-inflammatory medicine, heat, or corticosteroid injections into the effected joint. Repetitive gout should be treated with a combination of diet and with oral medication.

When fluid is examined from an inflamed joint in pseudogout, the specific causative crystal can be seen. Using shear wave elastography, we have for the first time, demonstrated that people with gout exhibit significantly lower stiffness in the AT compared with age/sex matched non-gout participants. Previously, Aubrey and colleagues reported that those with mid-portion AT tendinopathy demonstrated significantly lower mean SWE velocities than did those with a non-pathological AT. The loss of Achilles tendon stiffness we identified in those with gout was often greater than that demonstrated using a similar protocol in otherwise healthy runners with known Achilles tendinopathy. The mean SWE values determined by Payne et al ranged from 7.91m/s – 9.56m/s compared with mean SWE values in our participants with gout of 4.75 m/s – 10 m/s. Recent work suggests Achilles tendinopathy in healthy subjects involves both thickening of the tendon and softening , however our participants with gout did not report symptoms of AT.

However, treatment can decrease the duration of an attack and decrease frequency of future attacks. NSAIDs (i.e. indomethacin), corticosteroids (i.e. Prednisone) and colchicine are first-line therapies in cases of acute gout. Colchicine is less commonly used due to the potential side effects. Uric acid lowering agents are often used to treat chronic hyperuricemia.
Achilles Tendinitis Pathology
Gout is caused by abnormal metabolism of substances called purines that result in the accumulation of uric acid in the blood stream. When the excretion of the uric acid is hampered the accumulated uric acid in the blood stream causes crystalline deposits to form in joints or in the soft tissues. When this happens, there is a sudden onset of extreme pain with associated swelling, redness, and increased warmth to the skin or joint. Uric acid accumulation in other joints and areas of soft tissue is less common.
A thorough history and physical examination with assistance of advanced diagnostic tools and laboratory studies is essential to properly diagnose this condition. Differential diagnosis of gout should always be considered in patients with a history of hyperuricemia even if symptoms are masked by cardinal signs of infection. Oral corticosteroids can be as a methylprednisolone pack or prednisone starting at 40 mg or less, with a gradually tapering dose. Systemic steroids are the preferred agents in patients with renal failure in whom NSAIDs and colchicine are contraindicated.

Clinicians commonly use ultrasound imaging to visualise tendon structure in AT disorders, since the technique is minimally invasive, quick and often easily available for use . Shear wave elastography is a non-invasive and dynamic method of quantifying changes in tendon elasticity, involving application of a focused ultrasound pulse to the tissue being imaged .
Some studies estimated the prevalence based on radiographic findings of chondrocalcinosis in degenerative joint diseases; other studies estimated it based on synovial fluid analyses. This discrepancy creates limitations in extrapolating the data to patients with the clinical constellation of CPPD disease symptoms. Other adverse effects of colchicine include abdominal cramps, bone marrow suppression, axon-loss neuropathy, myopathy , potential liver toxicity, arrhythmia, shock, and skin rash . Concomitant use of colchicine with cyclosporine can lead to rapid-onset myopathy and increased myelosuppression. A 24-hour urine test for uric acid levels is necessary when considering a uricosuric agent.
Urate crystals may collect in the urinary tracts of people with gout, causing kidney stones. Some people may never experience gout signs and symptoms again. Medications may help prevent gout attacks in people with recurrent gout. If left untreated, gout can cause erosion and destruction of a joint. Low-dose aspirin and some medications used to control hypertension — including thiazide diuretics, angiotensin-converting enzyme inhibitors and beta blockers — also can increase uric acid levels.

This medication is injected subcutaneously by the patient once a day, usually for 3 days, but can be used longer if needed to resolve a flare.
