Pain started while sleeping at night, was somewhat better the next day, then excruciating pain since then. It showed up in the foot just below the big toe with swelling and redness. I am waiting on a blood test to measure the level of uric acid to determine the next step. It is important that damage to bone from gout be diagnosed, since documented damage is a clear indication for long-term therapy .

Can you massage gout away?
WebMD explains that while gout cannot be cured, it can be controlled with treatment. Anti-inflammatory drugs are one method, but in between gout attacks it can be helpful to receive massage therapy.
Following its introduction in the United States in February 1984, ESWL was rapidly and widely accepted as a treatment alternative for renal and ureteral stones. Anakinra (brand name Kineret®) is a biologic medication that blocks the inflammatory protein IL1. This medication is injected subcutaneously by the patient once a day, usually for 3 days, but can be used longer if needed to resolve a flare. Although much data supports the effectiveness and safety of this medication for gout, it is expensive and not as yet FDA approved for gout flares. It is still used off-label for gout, especially in hospitalized patients who often have risk factors that make the use of most other gout flare treatments more risky. Although exercise is great for helping lose weight and is generally fine for gout patients, when you have a gout flare in your toe, foot, ankle or knee it is advised to stay off the foot as much as possible until the flare resolves. It’s fine to do other kinds of exercise, for example any exercise involving the upper body, but give the gouty joint a rest.
Evidence suggests that eating a healthy diet with anti-inflammatory foods can ease RA symptoms. There isn’t a specific regimen to follow, but a 2018 review recommended the Mediterranean diet and fish oil for people with RA, Today reported. A small 2019 study by researcher Surabhi Gautam and colleagues published in Restorative Neurology and Neuroscience found that yoga eased physical symptoms and improved mental health. Excess urate results in deposition of uric acid crystals in joints and synovial fluid. This stimulates mobilization of polymorphonuclear leukocytes and other inflammatory mediators to the affected joints, most commonly the great toe or elbow, resulting in severe pain, swelling and an episode of gout. Several pipeline drugs for the treatment of gout include the selective uricosuric drug RDEA594 and various interleukin-1 (IL-1) inhibitors .

Intra-articular corticosteroids may not be feasible when many joints are affected. Prednisone in a dose of 30 mg twice daily is effective in most cases. Most often, symptoms start in smaller joints such as fingers and toes, and then move to other joints. Heat, pain, redness, swelling, and extreme tenderness that comes on quickly in your big toe joint may be caused by gout. Kidney stones should be removed by surgery if they cause repeated infections in the urine or because they are blocking the flow of urine from the kidney.
