What is the main cause of gout?
Gout is caused by a condition known as hyperuricemia, where there is too much uric acid in the body. The body makes uric acid when it breaks down purines, which are found in your body and the foods you eat.
The diagnosis of gout is made in the presence of 6 of the 10 criteria listed in Table 1. High blood pressure is another major risk factor for gout. It gets complicated, though, because the diuretics taken to lower high blood pressure increase uric acid levels, so the treatment as well as the disease is associated with gout. Gout may be secondary to sleep apnea via the release of purines from oxygen-starved cells. The most important differential diagnosis in gout is septic arthritis. The build-up of uric acid crystals begins with purines, a chemical compound found in many foods. All protein from animal flesh can potentially lead to elevated uric acid levels. Colchicine can reduce inflammation and pain if you take it within 24 hours of a gout attack. You may not have any of the classic symptoms of a gout attack. The exact cause of hyperuricemia sometimes isn't known, although inherited factors seem to play a role. Paying attention to what you eat may help you manage your gout. Eat moderate amounts of a healthy mix of foods to control your weight and get the nutrients you need.
Other symptoms of gout include warm, red skin around the affected joint. While it is reasonable to decrease or avoid these foods, it has been found that a high purine-rich diet does not increase the risk of gout, or aggrevate symptoms in research studies. This stage occurs when the urate crystals that have been deposited suddenly cause acute inflammation and intense pain. This sudden attack is referred to as a “flare” and will normally subside within 3 to 10 days.
While gout can be extremely painful, it is a very treatable condition. Most doctors will wait several days to weeks after a gout attack is over to begin medicine to lower the high uric acid levels. These medicines can cause uric acid stored elsewhere in the body to begin moving through the bloodstream and could make symptoms worse if treatment begins during a gout attack. The goals of treatment for gout are fast pain relief and prevention of future gout attacks and long-term complications, such as joint destruction and kidney damage. Treatment includes medicines and steps you can take at home to prevent future attacks. In the first stage, you have high uric acid levels in your blood, but no symptoms.
