If the blood uric acid is high but a person has never had an attack of gout, treatment is rarely needed. But people with extremely elevated levels may need regular testing for signs of kidney damage. And they may need long-term treatment to lower their uric acid levels. Your blood uric acid level may be watched by your doctor until it is lowered to normal levels. Additionally, your doctor will look at your uric acid levels. Extremely high levels of uric acid (greater than 12 mg/dL) may help your doctor with a chronic gout diagnosis.

Patients report the rapid development of severe pain ("worst ever") and tenderness that reach their maximum within 6-24 hours of onset. Sudden onset of pain, swelling, and erythema are characteristic of an acute gout attack. Joint aspiration can help distinguish from other types of arthritis and demonstrates needle-shaped crystals with negative birefringence under polarized light. Since imaging characteristics of gout are often diagnostic, joint aspiration is not always necessary. The pain may develop in the night and hurt continuously for up to two weeks before the flare subsides, with the most intense pain in the first 24 hours. But if gout is not treated, typically with medication to lower uric acid levels, gout flares will recur and over time and affect more joints, including the knee. Many people with high levels in their blood never get gout. But when uric acid levels in your blood are too high, the uric acid may form hard crystals in your joints. Uric acid is an end product of the breakdown of purines, compounds found in all body tissues and in many foods, such as liver, dried beans, asparagus, mushrooms, and anchovies. Uric acid is normally carried through the blood and eliminated in the urine. If production of uric acid increases, a person eats a large quantity of foods high in purines, or if the kidneys are unable to adequately eliminate uric acid, then concentrations in the blood can rise . When the crystals accumulate in the joints, they can cause the pain associated with gout.
See your doctor if you experience a suddenly painful joint. You canlower uric acid levels by limiting foods high in purines, and alcohol intake . The fluid withdrawn by an arthrocentesis is analyzed in a lab under a microscope for the presence of uric acid crystals. The fluid analysis can also rule out other causes of inflammation including bacterial infections.
How long does it take to trigger gout?
Uric acid is a waste product made by the body when it digests certain foods. When uric acid levels are high, crystals of it can accumulate in your joints. This process triggers swelling, inflammation and intense pain ( 5 ). Gout attacks typically occur at night and last 3–10 days (6).
Other medications that increase the risk include niacin, aspirin , ACE inhibitors, angiotensin receptor blockers, beta blockers, ritonavir, and pyrazinamide. The immunosuppressive drugs ciclosporin and tacrolimus are also associated with gout, the former more so when used in combination with hydrochlorothiazide. Uric acid-lowering therapy helps prevent future acute attacks of gout and gradually dissolves the uric-acid-containing tophi. Correction and prevention of hyperuricemia - Uricosuric drugs or allopurinol may be used alone or in conjunction. Hypouricemic therapy may also be used for patients with tophi and a higher recurrance rate. Dietary restriction of high-purine foods is a less effective treatment technique but is still recommended.
The annual incidence of acute attacks of arthritic pain and swelling is about 1.3 per 1000 adults, but nearly 50% of adults develop radiographic changes typical of CPPD by age 80 years. Several drugs have been implicated as possibly responsible for induction of CPPD. These include loop diuretics and proton pump inhibitors, both of which are associated with hypomagnesemia, and bisphosphonates. However, the role of these drugs in CPPD is controversial; they may even be protective.
Over time, lumps of uric acid crystals, called tophi, can also form around your knee. These lumps aren’t painful, but they can cause additional swelling and tenderness during a flare-up. Your doctor may also order a blood test to check your uric acid levels. Some people have high levels of uric acid and don’t develop gout. Others have typical uric acid levels but still develop gout. As a result, your doctor will want to do some other tests as well.
