Many patients have a lot of guilt associated with their disease—stemming from the myth that gout is self-inflicted from poor dietary and lifestyle habits. When lack of mobility affects work productivity, this reduces self-esteem and often limits participation in social and leisure activities or hobbies. The good news is that even in this late stage, several medications can be used. Nonsteroidal anti-inflammatory drugs – Several—like Advil®, Aleve® and Motrin®—are available in over-the-counter dosing. Other brand-name NSAIDs include Celebrex®, Indocin® and Naprosyn®.
Colchicine capsules are not a pain medicine and it should not be taken to treat pain related to other conditions unless specifically for those conditions. Even medicines that you take for a short period of time, such as antibiotics, can interact with colchicine capsules and cause serious side effects or death. Colchicine capsules can cause serious side effects or death if levels of colchicine are too high in your body. In a published study in healthy volunteers, 40 to 65% of the total absorbed dose of colchicine was recovered unchanged in urine. Enterohepatic recirculation and biliary excretion are also believed to play a role in colchicine elimination. Colchicine is a substrate of P-gp and P-gp efflux is postulated to play an important role in colchicine disposition.

Yet another study, a retrospective study found that initiation of ULT during acute flares led to more frequent flares in the first 12-weeks following the initial flare.14 Thus, initiating ULT during acute flares needs further study. Another principle of management that trainees often struggle with is the use of agents concurrent with ULT initiation to mitigate the risk of an acute gout flare. The American College of Rheumatology recommends treating patients beginning ULT with anti-inflammatory agents for at least 6 months after initiation of therapy or 3 months after urate goal is achieved or tophi resolve, whichever comes first. Given that non-steroidal anti-inflammatory drugs are relatively contraindicated in people with advanced CKD, the available armamentarium is more limited. The classic alternative, colchicine, also gives us pause when we consider its chronic use in the setting of CKD because of the risk of neuromyotoxicity and marrow suppression.

If acute gouty arthritis is suspected, arthrocentesis and synovial fluid analysis should be done at the initial presentation. A typical recurrence in a patient with known gout does not mandate arthrocentesis, but it should be done if there is any question of the diagnosis or if the patient’s risk factors or any clinical characteristics suggest infectious arthritis. Palpable tophi develop in patients with gout and can rarely occur in patients who have never had acute gouty arthritis. They are usually firm yellow or white papules or nodules, single or multiple.
